INFORMED CONSENT AND DISCLAIMER, AGREEMENT TO HOLD HARMLESS REGARDING COVID-19 AND CITY OF CHARLESTOWN, CHARLESTOWN PARKS DEPARTMENT PROGRAMS, EVENTS AND FACILITIES:
MASKS, SOCIAL DISTANCING & MANDATES: The State of Indiana has issued several mandates, guidelines regarding public events and facilities, and minimum health standard protocols. Indiana. Gov. Eric Holcomb's executive order requires Hoosiers age 8 and up to wear masks (covering both the mouth and nose) in all indoor public settings and outdoors when they cannot maintain 6 feet of distance from others.
The protocols apply not only to the operators of sports and parks facilities, such as the City of Charlestown, but they also apply to participants, families, and groups, and they may be amended. Everyone has a part to play in terms of responsibilities and safe practices. Whether you look at it as a health and safety matter - or simply one of common courtesy - please respect the space of others and wear a mask when indoors or in close proximity to other visitors and our staff. We understand this is a challenge for many children and acknowledge the implications of the outdoor nature of this event in that regard. Let's work together to keep things as safe as possible for everyone.
OUR COVID-CONSIOUS PLANNING PROCESS: Our staff has worked closely with the Clark County Health Department over the last year to plan all activities in a COVID-consious manner, placing the health and safety of our staff and community at the forefront in all planning and decision making. Health Department approved facility and event plans are available for review upon request. Here are a few of the steps we're taking to keep our community and staff as healthy as possible:
🐰 Temperature Checks
🐰 Hand Sanitizing Stations Throughout the Park
🐰 Encouraging Social Distancing, Masks Required at City Facilities
🐰 Smaller Groups - More Space, Less Crowd
🐰 Sanitizing and Cleaning Between Sessions
PROGRAM SUBJECT TO CHANGE: At the time of this notice, COVID-19 numbers have continued to trend downward in our area, allowing for increased capacity at our facilities and events. The terms of this event are subject to change at any time in accordance with any mandates or directives from the State Department of Health or the Clark County Health Department, or to preserve the wellness of our staff and community. Please monitor our social media pages as well as your email for updates regarding any changes made to, or impacting the format of this program.
WHEN WEIGHING WHETHER OR NOT TO ATTEND THIS VOLUNTARY RECREATIONAL EVENT, PLEASE CONSIDER THE UNIQUE NEEDS OF YOUR FAMILY. This may include but is certainly not limited to, consideration for high-risk health factors that place any member of your party in an elevated risk category. The City of Charlestown and, where applicable its operators and licensees, are following all state and local mandates and protocols, and have put in place preventative measures to reduce the spread of COVID-19; however, no guarantee can be made by any party that you or your child(ren) will not be exposed to COVID-19 in a public setting such as this. Further, by attending any program or event or using any city facility, you may increase your child(ren)s risk of contracting COVID-19.
ACKNOWLEDGEMENT OF RISK, AGREE TO HOLD HARMLESS:
By completing this registration form, I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or members of my group or organization, including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that they or I may experience or incur in connection with participation in any event, program, activity, reservation or rental taking place at a City of Charlestown Parks and Recreation facility.
I hereby release, covenant not to sue, discharge, and hold harmless the Town, its employees, agents, and representatives, of and from any claims related to COVID-19, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I further agree to indemnify and hold harmless the City of Charlestown and its employees, agents, and representatives from any claim that may arise from or in connection with my or members of my group or organization’s participation in any program taking place at a City of Charlestown Parks and Recreation facility, including claims related to COVID-19.
